Name order - We follow however it is spoken in the JP dialogue.
Honorifics - Our rule is Japan / Japan-like setting (think Virgin Road) we use honorifics. Foreign country / other world we do not.
This one was a bit odd because there were multiple Japanese people isekai’d to a fantasy world, so it has its own set of rules:
Royalty: Address by title. Princess Celia, for instance. Does not matter who is talking.
Isekai’d people: When addressing someone higher than them (-sama, -dono, etc), use title. -san, -kun, -chan stay.
Non-isekai’d people: Never use honorifics.

Do Chihiro’s releases have honorifics in their subs? What about name order?
